Job Purpose: In adherence to the specific task he/she is assigned to facilitate the daily implementation of the Targeted Supplementary Feeding Program, focusing on the screening, admission, and treatment of Moderate Acute Malnutrition (MAM) while providing essential nutrition and hygiene education to caregivers, the TSFP Facilitator carries out sensitization as well as anthropometric measurement or beneficiary registration, referrals, and discharges or ensures distribution of complementary food /as per the guideline/, participates in Post Distribution Monitoring Survey for U5 children as well as Pregnant and Lactating Women.
Major Responsibility
- Makes sure that the beneficiaries are seated comfortably in the waiting room;
- Explains the nature of the program / its name, why it exists, whom it serves and what is required of them, what it does and why, the work procedures followed/, the nature of the complementary product/ product type and quantity, its nature, the target group, preparation utilization and storage and related hygiene requirements/ as well as program schedule;
- Notifies crowd controllers what to do to manage beneficiaries’ subsequent screening activities;
- Check that all the needed materials/equipment are ready and in good working condition;
- Checks eligibility of beneficiaries /whether they are U5, PW, or LW/ registration details/whether they are new or re-referred from other programs or already enrolled in the program, appropriateness of site and schedule of service screening status /whether they are already screened or not;
- Check the presence of bilateral edema and, if any, fills out the internal referral form fully and correctly and send it to the registration point; If the beneficiary is not oedematous, carry out MUAC measurement of children and PLWs;
- Re-Checks nutritional status of U5 children /who are at risk/by taking weight /with 0.1kg precision/ and height /with mm precision/ measurement;
- Fills out the internal referral form with all relevant information fully and correctly and forwards beneficiary to subsequent Registration Point;
- Eligible children: registers the new beneficiary, fill out all required information on the master beneficiary list, issues Beneficiary identification card are completed and forwards beneficiary to Distribution Point;
- Collects and keeps Beneficiary Identification Card, fills out the external referral form in two copies, and refers the beneficiary with one referral form to an appropriate treatment program;
- Explains reasons od admission or discharge beneficiary, collects beneficiary card, puts necessary remark on the master beneficiary list
- Provide on-the-job training and support according to areas to improve on identified gaps for incentive workers. Receive and ensure materials, formats, and equipment is needed for daily activity.
- Prepare periodic reports (daily/weekly/monthly) on a regular basis, as appropriate and following prescribed format to the immediate superior;
- Resolves any problem/challenge in consultation with immediate superior
- Ensure good coordination and collaboration with other health and nutrition team members and support departments
- Ensure proper and economic usage of all resources under his/her custody;
- Represent immediate superior, when delegated;
- Performs other related tasks as and when assigned by immediate superior
